The Use of Facility Form is provided for outside organizations to request use of the FYC Property. All Use of Facility Forms should be completed and returned to the Firth Youth Center at a minimum of 30-days prior to the request. Requests will not be considered until the required Certificate of Insurance is provided. Details of this requirement is outlined in Section B of the Form. The FYC Board of Directors reserves the right to deny any and all requests to use the facility and to waive fees.
